Complications and sequelae

• Presentation at EAG call and discussions with @jca and @pbr

• Proposal to retire complication and sequela and model/name all combination concepts using current guidelines for combined disorders/disorders+procedures while adding complication or sequela as additional descriptions.

• Our general heuristic for diabetes complications is to make these co-occurrent and due to. This results in them inheriting a parent of 237620003 |Abnormal metabolic state in diabetes mellitus (disorder)| whether or not the complication is an abnormal metabolic state.

Complication of pregnancy, childbirth and/or the puerperium (disorder)

• It is suggested that |77386006|Patient currently pregnant (finding)| be reviewed for retirement.

  • "Intrauterine pregnancy (finding)" is not a “Patient currently pregnant (finding)”, it is a "Pregnancy finding". This seems to be an inappropriate parent not only for this concept but for many others. "Patient currently pregnant (finding)" has descriptions like “ Pregnancy confirmed” but has children like Pregnant - on history (finding).

Potential values for temporal relationships related to pregnancy, childbirth and the puerperium discussed. It was noted that the current potential values sound awkward when used with their associated attributes but it was noted that these relationships would not be visible to the end user and thus be used as long as it was clear what the values represent. It was felt therefore that  |77386006|Patient currently pregnant (finding) could be clinically useful and should be retained. The possible use of 307159006 |Pregnancy time period (qualifier value)| with occurrence or a new role was re-examined. No decisions were made. There was agreement that this are requires further analysis and discussion.
