Welcome and role call

BGO, all

@Bruce Goldberg offered apologies. Still not able to start meeting.

2

Allergy model

BGO

  • Follow-up discussion on allergy modeling

    • Modeling with has realization Allergic reaction (disorder) vs.

    • Modeling with has Realization Allergic reaction process (qualifier value) vs.

    • Modeling with has realization Allergic reaction to X (disorder)

  • Can Allergic disposition contain a Pathological process relationship?

  • has Realization Allergic reaction process (qualifier value) does not require extending the range of has Realization to include clinical findings/disorders and does not require pre-coordination of a large number of allergic reaction to X concepts. The latter however may be clinically useful

    • Test all 3 models in SCA UAT

  • Awaiting analysis from Stefan Schultz

3

Updates to editorial guidance on combined disorders

BGO

  • General heuristics for diabetes complications

    • Recommendation that for majority of complications related to type I/II DM use X co-occurrent with Y pattern. Example terming: FSN - Retinopathy co-occurrent and due to type I diabetes mellitus (disorder); PT - Retinopathy with type I diabetes mellitus

    • For exceptions such as complications related to drug-induced diabetes mellitus or gestational diabetes, there is the option to use the due to pattern

4

New modelers test for combined disorders

BGO

  • Testing advantageous in order to identify general patterns for modelers to apply

  • @Bruce Goldberg to create subset of examples from @Phil Brown for which he and @Bruce Goldberg did not agree.

5

Complication of pregnancy, childbirth and/or the puerperium (disorder)

BGO

  • Review of proposed values for temporal sequence roles

  • 77386006 |Patient currently pregnant (finding)|

  • 23667007 |Term pregnancy delivered (finding)| or 289256000 |Mother delivered (finding)|

  • 86569001 |Postpartum state (finding)| (synonym puerperium)

  • @Bruce Goldberg to model examples using various temporal sequences with above values

6

Exacerbated by

BGO

  • The periodontal community will likely be changing some of their disorder descriptions to include the phrase "exacerbated by".

Though I have no precise examples, it might be something like "severe generalized periodontitis exacerbated by uncontrolled diabetes" or even possibly a procedure such as "severe generalized periodontitis exacerbated by chemotherapy".

There are is an existing concept in SNOMED that use this phrase e.g. 707245003|Angular cheilitis due to nutritional deficiency (disorder)| with a synonym of "exacerbated by".

An unapproved qualifier exists of 410660005|Aggravated by (attribute)| that has a synonym of "Exacerbated by".

  • Two options for modeling discussed:

    1. Use of unapproved attribute aggravated by (attribute)

    2. Exacerbation of X (co-occurrent and) due to Y

      1. Requires creation of additional Exacerbation of X (disorders) which are currently for the most part primitive leaf nodes

  • Option 2 approved. @Penni Hernandez to provide examples from periodontal domain for testing

AOB, adjourn
