  • Aspiration pneumonia resulting from a procedure (disorder). This is a complex concept containing a combined disorder (aspiration pneumonia=Pneumonia due to Aspiration into lower respiratory tract (finding) + RG(associated morphology=inflammation and consolidation)+finding site=lung structure) which is due to an unspecified procedure. There needs to be a mechanism for associating the Due to roles separately for the procedure and the pathological condition. A proposed model has due to procedure in role group 0 and due to the condition of aspiration pneumonia in a role group with the associated morphology and finding site. Editorial guidance in terms of which patterns identified in the FSN will require this approach needs to be developed. The impact of implementing this model will also need to be assessed.

  • Should procedures be a direct cause of their complications?

Example: Infection due to gastric band procedure. Is this correct?

  • Question of whether proposed model, grouping causative agent with pathological process of allergic process and moving after-→allergic sensitization to role group 0 still captures the intended workaround for indirectly associating a causative agent with a disposition. An alternate model was proposed to change pathological process--→allergic process (qualifier value) to pathological process--→allergic sensitization process (qualifier value) in role group 1 and converting Allergic sensitization (disorder) from fully defined to primitive by removing pathological process--→allergic sensitization process (qualifier value)from the definition of Allergic sensitization (disorder). This latter recommendation is made in order for Allergy to X not to inherit, Allergic sensitization (disorder) as a parent.

  • Proposal: Procedure complications temporally or causally related to procedures for which the complication is a combined life phase containing a condition which can be represented as a pathological structure and a temporal or causal relationship to a life phase containing a second condition will be modeled with the temporal or causal relationship to the procedure in role group 0 and the temporal or causal relationship to the resultant condition in a role group with the pathological structure.

Combined disorders causally related to > 1 link on a causal chain

Diffuse large B-cell lymphoma due to chronic inflammation. This interesting disorder apparently occurs in cells infected with EBV.

Primary effusion lymphoma is due to HIV in cells infected with HHV-8

Additional goals for 2017

BGO

  • Complication of pregnancy, childbirth and/or the puerperium (disorder)

  • Complication associated with device (disorder)

  • Combined disorders with events

  • Pregnancy, childbirth and puerperium are observable (function) entities

    • Should there be related findings? If not how to related complication (disorder) to an observable?

    • Use of new temporal roles (during, before, temporally related to vs. descendants of 307159006 |Pregnancy time period (qualifier value)|

  • Complication associated with device (disorder) is a child of Complication of procedure (disorder) yet the descendants of the former appear to be complications related to the device itself rather than the procedure used to place the device

  • Example:

Accidental needle stick with exposure to body fluids (disorder)
