The nutrition concepts from the SNOMED Jan. 31, 2021 release were reviewed. All of the concepts are in the January 2021 release. We appreciate everyone's participation in reviewing the items.

Concept approval process: Steps in SNOMED review and approval

The concept review process was described. Editors review the authored concepts to ensure compliance with SNOMED's editorial standards. Quality checks in the tool authoring ensure consistency with authoring standards. Computer classification and validation ensure that the content fits with existing and newly authored SNOMED concepts.

International Dysphagia Diet Standardisation Initiative (IDDSI) update

IDDSI leadership have reached out to staff to ask about the NCPT work ongoing by the Academy. They are working to understand how they might engage with SNOMED in the future.

Three models were reviewed.

Increased nutrient needs (NI-5.1). The group supported:

  • Accepting current modeling

  • Submitting Increased nutrient need (rather than ‘needs’) as synonym

  • Query about the FSN, which uses 'nutritional' vs. 'nutrient' in the model

Altered nutrition related laboratory values (NC-2.2)

  • For some members, this concept is used and interpreted as biochemical laboratory data.

  • Use case ideas that might also be used could include bioelectrical impedance analysis (BIA), scanning indicating fatty liver, fecal fat.

  • The recommendation is clarification of the concept and definition by the NCPRO Committee prior to further work on the mapping and modeling in SNOMED CT.

Inadequate protein energy intake (NI-5.2)

  • Members suggested that inclusion of this concept in NCPT likely stems from its inclusion as a concept in ICD-10. This fits with its initial placement immediately preceding Malnutrition (which is now in the clinical nutrition diagnosis domain). The existence of Protein calorie (or energy) malnutrition in ICD-10 classification serves a different purpose than that of SNOMED CT which supports clinical documentation.

  • Members suggested that the use of this concept, whose definition relates to protein and energy intake, may be better accomplished by those separate nutrition diagnosis concepts.

  • The recommendation is clarification  by the NCPRO Committee of the concept and its use prior to further work on the mapping and modeling in SNOMED CT.
