Perioperative complications



















Interim proposal to model combined disorders with a causal association between conditions

  • Discussion regarding meaning of perioperative

    • SSH felt that meaning should just include during and or after

    • Anesthesia SIG felt that perioperative includes before surgery as well

    • Question arose as to whether an undesired effect that occurs before a contemplated surgical procedure and prevents the procedure from being carried out would be considered a preoperative complication. Group consensus was that the surgery would need to have been undertaken for a an undesired/unanticipated outcome to be considered a preoperative complication.

    • SSH felt that anesthesiologists may consider the surgical episode as starting with anesthesia while surgeons may may consider the surgical episode as starting with the initial incision

    • Naming of a proposed new attribute ((before) during and or after, surrounding, encompassing, temporally related) was discussed. Name should convey actual meaning which would be understood for different languages.

    • Complications of procedures imply the complication is due to a procedure. Perioperative complications on the other hand just mean that the complication is temporally related to surgery and not necessarily due to surgery. Currently perioperative procedure is a descendant of complication of surgery. Recommendation was to make these siblings by defining Complication of procedure as due to procedure and perioperative complication as encompassing, etc. surgery (see updated owl file).

 

  • Group felt proposal should just be applied to ICD-11 harmonization work

Authors combined disorder modeling test

  • Results of author's test reviewed (summary appears in attached spreadsheet)

  • Difficulty in discerning between co-occurrent and due to and due to discussed


Will create new test with instructions to apply the following heuristics:

 

  1. For all resultant conditions that represent manifestations of a multisystem disorder, model as co-occurrent and due to.

  2. For all causative conditions that are systemic and non-curable, model as co-occurrent and due to.

  3. For all combined disorders where a cause can be either a disorder or a material agent (e.g. gallstones), model as caused by material agent.

  4. Use pattern 1, simple co-occurrence very sparingly - only for 2 conditions that are related by a common predisposition or represent an overlap syndrome.

  • Will use SCA UAT as can create a separate branch which can be moved to the production environment if testing is successful
