    • Secondary disorders subsume a variety of concepts in which a disorder is causally related to other disorders, procedures, material agents and physical forces.

    • Defining such secondary disorder heading concepts would require use of GCIs

  • Drug overdose

    • Further discussion required re. overdose finding as an abnormal (elevation) of a drug on a laboratory test may not be due to an overdose but rather to a metabolic abnormality.

Need to estimate project size
Advised to confirm that anesthesia SIG comfortable with preoperative complication as during and/or after surgery as opposed to before, during and/or after surgery.
At any rate, need to add before as an additional attribute to model e.g. preoerative anxiety

 

 

 

As GCIs not supported by current SNOMED DL (and unlikely to be implemented in near future) it was suggested that secondary x could be modeled using domain-specific templates (or a general template) in protege and classified descendants then added to SNOMED CT. Yong will investigate. He will post his OWL file.

 

Currently only two drug overdose concepts reside in the events hierarchy. It would require a large effort to move the existing disorder content under events. It was felt that clinicians do not distinguish between an overdose event and overdose disorder. Recommendation is to leave overdose concepts in disorder hierarchy. It was agreed that an elevated drug level is not necessarily synonymous with an overdose.

Further revisions to editorial guidelines were reviewed and approved by the group.

Examples from authors test involving systemic disorders with additional disorders that may be inconsistent features of the sytemic disorder e.g. Disorder of respiratory system due to dermatomyositis (disorder). Discussion centered around whether these should be co-occurrent with vs, co-occurrent and due to. No decision made. Further discussion needed. Suggestion made to collect examples in which a distinction may need to be made between due to and co-occurrent and due to.

Allergic conditions

  • Allergic diseases caused by specific allergens are represent in SNOMED CT as conditons (dispositions or processes) but modeled as if processes with causative agent

  • Should allergic disease x caused by y be considered to be processes only and if so, how to represent the disposition?

  • Relationship between an allergic disposition allergic process (reaction) and history of allergy

Background leading to development of SDP allergy model discussed. It was realized that this domain is a special case and may not not generalizable.
Many allergic disease x due to y concepts modeled with a due to allergic reaction to Y. This role needs to be removed after analysis of impact on inheritance.
It was reiterated that the retirment of history of allergy to x was warranted due to the potential confusion between an allergy disposition and allergy resolution.
