Statement of problem as requested or initially identified

  1. 160244002|No known allergies (situation)| is plural; it should be singular.

  2. Naming differences of subhierarchy:

  •

    •

      • no known x allergy

      • no x allergy

      • no past history of x allergy

      • no known history of x allergy

  1. 401189005|No past history of animal allergy| has an associated finding of |Dander (animal) allergy|.  However, in addition to dander, animal allergy could include feces, urine, and saliva.

  2. 428197003|No known insect allergy| has an associated finding of |Allergic disposition|.

Relevance to International edition

None identified.

Solution proposed

  1. Inactivate plural 160244002|No known allergies (situation)| and create a new singular concept |No known allergy (situation)|.

  2. Ask the Editorial Panel for terming guidance for the subhierarchy, currently:

  •

    •

      • no known x allergy

      • no x allergy

      • no past history of x allergy

      • no known history of x allergy

Once a naming convention is approved, correct the relevant FSNs by inactivating the current FSN and creating a new one.

  1. Correct 401189005|No past history of animal allergy| associated finding of |Dander (animal) allergy| by creating a new Clinical finding concept “Allergy to animal” with the causative agent |Animal material (substance)|.  Make the concept fully defined, as there would be concepts that would be its subtypes.

  2. Correct 428197003|No known insect allergy| associated finding of |Allergic disposition| by creating a Clinical finding concept |Allergy to insect (disorder)|. Make the concept fully defined, as there would be concepts that would be its subtypes.

Stakeholder input

Solution #3 input:

Romin Khazai, IHTSDO

I think this whole area of the substance hierarchy needs some work. For example, what is the difference between animal material and animal agent? In the meantime, a causative agent to animal protein sounds like it would work.

Bruce Goldberg, KP

It is correct in that animal allergy can refer not only to dander and epithelia but to feces, saliva, serum and urine proteins. You could create a new concept of allergy to animal. I would use a causative agent of animal protein, as animal allergens would tend to be proteins and not the living animal itself.

Solution #4 input:

Romin Khazai, IHTSDO

Insect allergen (substance) only has insect venom (substance) and a couple of other concepts underneath, but if insect protein (substance) has the whole range of concepts, which may make the concept more comprehensive.

Bruce Goldberg, KP

For insects, similarly to animals, allergy to insect can be created with a causative agent of 710942001 | insect protein (substance) |.  Insect allergens consist of whole body extracts, various body parts, salivary proteins, feces and venom.  406470001 | insect allergen (substance) |as you note is also available but I think there is some reluctance to using the allergen class hierarchy.

Impact assessment

Inconsistent naming of concepts and the animal/insect allergy modelling will be corrected.

Risk assessment

Inaction will prolong the inconsistent naming of concepts. The animal/insect allergy modelling will be incorrect if not corrected.
