Changes to substance and product groupers to support allergy and adverse reactions

• Includes the attached commentary regarding anesthetic agents requested from this group as well as a proposed hierarchy of contrast media agents

• The majority of the concepts related to "Allergy to role grouper" and "Allergy to structure + role grouper" have been inactivated for the July 2020 release in consultation with the Allergies CRG. The remaining concepts in the two categories noted above (~10) are planned to be modelled with "product representing role" and "product representing role + structure". There are two dependencies in making this change:

•  

  • EAG/MAG decision on how these concepts need to be classified--specifically where there is an intersection between substance and products (Yong's presentation will elaborate on this topic)

  • Identifying the required sub-groups for a couple of role groupers, namely "Anesthetic" and "Contrast media" (Lists of proposed groupers have been submitted)

Modeling of Allergy/Adverse reaction to substances/products

• Proposed models presented to EAG and MAG for modeling allergy to products including combination medications and vaccines

• Need to be able to document allergy not only to active ingredients but also for the rare instances to products for which the allergy may be to an inactive ingredient

• Above requirement may be best supported as allergy to substance and allergy to product being siblings

• Agreement that allergy to combination products would best modeled using GCIs

• Agreement that allergy to vaccines would be best modeled with causative agent product

Allergy to animals

• Proposed model for representing allergy to animals and animal-derived substances

• Needs further discussion in terms of how to relate allergenic substances (e.g. proteins) to sources such as dander to whole organisms
