Allergy content changes for July 2019 release

Templates have been developed based on final modeling decisions and are ready for technical implementation. [ https://confluence.ihtsdotools.org/pages/viewpage.action?pageId=64260419 ]

The template based transformation function will be available in middle of August. The content allergy to X and allergic reaction to x will be transformed by using the new function in the authoring tool.
