2022/09/28 - SNOMED CT - Making a difference in primary care (Lisbon)
Primary care provides the life long record of a patient in terms of the care, medication and health issues. To obtain the greatest value for this data, it is essential that the information is in a format which can be shared and reused. SNOMED CT provides an international standard for recording health data in both primary and secondary care, allowing the maximum benefit to be obtained. It also provides interoperability between other standards used in primary care. The presentations included in this session highlight the requirements and also some of the solutions that are central to the delivery of primary care in todays challenging times.
