Previous lead of nursing CRG works for Cerner and wanted to share with us work they are doing re SDoH.

Developing a framework for linking different elements, with framework including:

• Sociodemographics – age, gender, race, ethnicity, education, employment 

• Psychological – health, literacy, public engagement, self efficiency (includes levels of stress, depression, anxiety ) 

• Behavioural – nutrition, physical activity, tobacco use, tobacco exposure, alcohol use, substance abuse, sexual practices 

• Individual level social relationships – social connections, social isolation, exposure to violence, social support, work conditions, incarceration, military service 

• Environment – air, water, built environment, housing 

• Financial resource strain – housing, food, transportation, utilities, income, childcare etc

They have 8 tools defining SDoH in US, 6 of which include the following: 

• Diagnosis and treatment

• Care coordination

• Risk assessment

• Appropriate referrals

• Patient experience 

• Research and quality

• Data encoded for multiple usage, regardless of data source

• 3 tier equivalency scoring
