Working with NHS England to identify social needs post first COVID lockdown.

Attempting to draw out indicators form reviewing GP data – focus in food insecurity, housing, finance, domestic abuse etc. 

Identified a list of SDoH in SNOMED CT and looking for these indicators in GP data – started with Read Codes and now also including SNOMED CT. 

Want to track any social needs and the corresponding social interventions/activities provided. 

Provided a minimum dataset for information – currently in draft. Demographics, presenting problem, interventions and want to add `adherence persistence and outcome 

Move to social prescribing – system call Elemental
To send us indication of gaps.

Also aware of other services using similar entities e.g. police and councils
