Additional Relationships

The new "additional relationships" take the same form as the inferred relationships and can be stored in the same format in the RF2 package. Modeling Advisory Group (MAG) recommends that extensions can use the existing relationship files or create new relationship files for them. By default, there is a single file for each component (1 concept file, 1 relationships file, 1 simple refset file, etc.). Edition and extension publishers can decide to split the content of any of those files for business reasons while following the naming convention. The new attributes should be placed under 1295449009 |Additional relationship attribute (attribute)|. The concept 900000000000227009 |Additional relationship (core metadata concept)| should be assigned to characteristicTypeId in the relationship files.

 The concept model for fetal procedures and findings has been updated in the international edition of SNOMED CT. The model employs the attribute 246454002 |Occurrence (attribute)| with the value 303112003 |Fetal period (qualifier value)| and its subconcepts for modeling purposes. Finding and procedure sites have been revised to utilize canonical or morphological structures (excluding unique fetal structures) such as persistent or malformed structures. For example, concepts modeled with site 'fetal head structure' have had site replaced by 'head structure' in combination with 'fetal period' occurrence.

The 106 descendants of 367570001|Fetal part (body structure)| are currently under review. Non-unique fetal parts, such as ‘fetal head’, will be inactivated from the anatomy hierarchy. A briefing note will be published before the inactivation of anatomy content. National extensions should update the model for fetal findings and procedures to ensure consistent classification. 409793007|Antimicrobial resistant bacteria (organism)| and descendants will be reviewed and content corrections made as appropriate.  The review will focus on:

• Aligning taxonomic lineages of resistant bacteria with those of bacterial (only) portions of the organisms hierarchy. Missing ranks including (and limited to) order, family, genus, species will be added as necessary.  Example:

  • The lineage of beta-lactam resistant Escherichia coli, beta-lactam resistant Escherichia (genus), beta-lactam-resistant Enterobacteriaceae (family), beta-lactam-resistant Enterobacterales (order).  

• Aligning organisms that are said to be resistant with corresponding antimicrobial substances hierarchy.  Example: 

  • Ampicillin < penicillin < beta-lactam

• Making the grammar and description style consistent throughout the hierarchy and compliant with the editorial style guide. The pattern “antimicrobial-resistant organism” will be maintained.  The antimicrobials themselves may be hyphenated.

Proposal to Create New Attribute to Link Substance Hierarchy to Organism Hierarchy

Feedback is required on the level of support from the SNOMED community for the launch of a project – for further details please see this briefing note.

Project Update 27 August 2026

The attribute has now been created; however, its name and definition remain subject to refinement as part of the project. The initial work to create linkages between concepts in the Substance and Organism hierarchies, within the limited scope of microorganism nucleic acids and antigens, will be undertaken by Australia and will be subject to internal review and validation.

The attribute and the associated linkages will be promoted once the project work is complete and has been fully vetted.

A review of “sepsis caused by organism” concepts is underway. Please see this updated briefing note.

The proposed changes include:

• Inactivation and replacement of 80 concepts where significant updates to the FSN are required. Concepts will be inactivated with the reason “Outdated” and linked to replacement concepts using the historical association “Replaced by”.

• Replacement of the use of 246075003 |Causative agent (attribute)| with 42752001 |Due to (attribute)| linked to <<40733004 |Infectious disease (disorder)|. This reflects the current understanding that sepsis results from a dysregulated host response to infection, rather than direct organ dysfunction caused by the infectious organism itself.

• Updated description patterns for replacement concepts using the format: “Sepsis due to infection caused by X organism”.

• Review and inactivation of non-synonymous descriptions, where required.

These changes are expected to improve subtype classification consistency, particularly for bacterial and viral sepsis concepts, while aligning terminology content with current clinical guidance.

Use of ‘Tumor’ in Morphology and Disorder Concepts

Work is underway to review and address inconsistencies in the use of the term ‘tumor’ in SNOMED CT disorder concepts. 
The term tumor may refer to any mass (neoplastic or non-neoplastic); therefore, neoplasm is preferred, while tumor is acceptable as a synonym. Initial analysis has identified variation in usage. Some terms appropriately reflect histologic classifications (e.g. neuroendocrine tumors aligned with WHO Blue Books), while others appear to represent legacy, outdated, or inconsistent naming.
450 disorder concepts with tumor in their FSN are being examined and will be updated where appropriate. The work is being undertaken for this Content Tracker.

Revision of Requested, Done, Received, Planned Concepts

Work is underway to address inconsistent modelling of request- and workflow-related content currently represented under Clinical finding. Concepts representing requested procedures (e.g. domiciliary visit requested) are valid but will be aligned to the Situation with explicit context hierarchy. In contrast, concepts representing event status (e.g. done, received) reflect outdated naming patterns that are no longer accepted, and along with report status concepts represent administrative workflow rather than clinical meaning and will be inactivated.

• X visit done/received/planned (n=29): Inactivated (administrative workflow / event status)

• X visit requested (n=26): Moved to Situation with explicit context hierarchy, with supporting procedure concepts created

• X report status (n=50): Inactivated (administrative workflow content)

This work improves alignment with editorial policy and ensures appropriate separation of clinical meaning from administrative processes.

LOINC Observable Entity Content Promotion

Observable entity concepts from the LOINC Ontology will be promoted to the International Release of SNOMED CT when needed to define clinical findings or when meeting the guidelines outlined in “Appendix C: Process for Addition of Observable Entity Content” of the Customer Guidance for Requesting Changes to SNOMED CT.

• An annotation with an attribution will be added to these concepts. The format has the name of the originating organization and a canonical representation of the URI: “Regenstrief Institute: http://loinc.org/code”. Example: Regenstrief Institute: LOINC - LOINC 88462-7 Carbapenem [Susceptibility] .

• Descriptions originating from LOINC, e.g., LOINC long common name, will be removed during the promotion. The PT will be updated from the LOINC long common name to a SNOMED CT compliant description.

Cell Structure

Addition of Human Gene Content

Human genes and their associated chromosome bands are being added to the International release of SNOMED CT in accordance with the specifications outlined in the briefing note circulated to the SNOMED Community. HGNC-approved names and symbols — along with previously used and alias symbols — are also being incorporated into gene concepts.

Gene additions will be prioritized based on known use cases, beginning with those required to support the ongoing development of the LOINC extension. Future use cases include support for molecular diagnostic findings and genetic disease modeling. Given the large number of genes involved, additions will be phased across multiple releases.

Incremental improvements in the laboratory content in the observable entity hierarchy will be made starting with the June 2026 release, including:

A review of approximately 300 nursing-related clinical finding concepts is underway to improve consistency of descriptions, synonym usage, and modeling for concepts containing terms such as “disability”, “reduced”, “limited”, and “limitation”. This work is part of this content tracker.

The proposed changes include:

• Removal of non-synonymous synonyms and creation of replacement concepts where required

• Standardization of disability-related descriptions and wording

• Review modeling patterns, in particular, the modeling of <<21134002 |Disability (finding)| to better reflect the concept definition

• Review and potential inactivation of ambiguous disability concepts

The work aims to improve semantic clarity, support consistent terminology modeling, and better align disability concepts with international definitions and clinical usage.

Based on a member inquiry and a subsequent review of subtypes of 46742003 |Skin ulcer (disorder)|, modeling inconsistency has been identified in the assignment of finding sites to ulcer concepts on external body surfaces i.e.. skin-bearing parts of the body.

This affects over 800 concepts and requires a coordinated editorial decision before remediation can proceed. The work item addressed inconsistencies in modeling ulcer finding sites, particularly whether ulcers on skin-bearing regions should be classified as "Ulcer of X" or "Ulcer of skin of X," focusing on standardizing terminology and structure.

As part of the addition of human genes to the SNOMED CT cell structure hierarchy, the existing 274 descendants of 260071009 |Human leukocyte antigen allele (substance)| will be inactivated and replaced with corresponding cell structure concepts.

Each replacement allele concept will be modeled as a descendant of the appropriate HLA gene structure concept and renamed to align with the current HLA allele nomenclature maintained in the IMGT/HLA Database, which contains the official allele names recognized by the WHO Nomenclature Committee for Factors of the HLA System.

Clinical finding concepts that reference these alleles will be updated to reflect the revised nomenclature and the associated concept model changes. This work is limited to replacing existing content; no additional HLA allele concepts are planned unless a specific clinical use case is identified.

Three attributes have been added to the MRCM for physical objects:

3. 434711009 |Specimen container (physical object)| will be sufficiently defined using the Is receptacle for attribute. 

4. << 337386000 |Test tube (physical object)| and << 706046003 |Specimen receptacle (physical object)| will be remodeled as subtypes of 706437002 |Container (physical object)| 

5. The physical object hierarchy will be reviewed for any other concepts that should be moved to 706437002 |Container (physical object)|, e.g. 469844003 |Ampule (physical object)| 

6. Around 180 concepts in the 706437002 |Container (physical object)| hierarchy will be remodeled using the three new attributes, along with Has compositional material and Is sterile.  

7. Around 70 new concepts requested by the eX-EHR project will be added. 

8. All concepts from other hierarchies that are impacted by this change will be reviewed. A preliminary impact analysis identified the following numbers of concepts per hierarchy that are defined using a concept that may be a container: 30 procedures, 26 of these have an attribute using device = cytology brush, 4 clinical findings and disorders. These refer to a catheter type, which would most likely not be affected by the remodelling of container types.

Content review and updates will standardize the classification of fetal orientation, addressing the ambiguity between normal pregnancy findings and clinical disorders during labor.

The changes will focus on:

Standardizing descriptions for fetal orientation:

• Presentation: The fetal part at the lower uterine pole (e.g., vertex, face, breech).

• Position: The relation of the presenting part to an anatomic axis (e.g., occipitoanterior).

• Lie: The relation of the fetus to the long axis of the uterus (e.g., longitudinal, transverse).

Making a clearer distinction between finding and disorder - orientation is dynamic during pregnancy (finding) but often transitions into a clinical concern during labor (disorder). Modeling approach:

• Finding concepts (e.g., Malposition of fetus) will be modeled with the occurrence of 1156673006|Fetal antenatal period (qualifier value)|

• Disorder concepts (e.g., Obstructed labor) will be modeled with the occurrence of 1156682000|Maternal intrapartum period (qualifier value)|

• Clinical disorders will be explicitly modeled as being "due to" specific clinical findings, effectively using these findings to define the disorder concepts.

These changes are planned because not all subtypes of morphinan and morphinane alkaloid are plant isoquinoline alkaloids or isoquinolines; they can be fully synthetic, semi-synthetic compounds derived from a plant alkaloid, salts of these compounds, or metabolites formed in the human body.

766762007 |Morphinan (substance)| will be added as a parent to 387114001 |Dextromethorphan (substance)|. This is replicated in its modifications, and will impact approximately 50 related findings/disorders, medicinal products, and clinical drugs.

Congenital abnormal fusion concepts that reference only a single bone will have their FSN and PT descriptions updated to clarify that they represent involvement of the specified bone in a congenital abnormal fusion rather than a complete statement of the structures involved in the fusion. Accordingly, FSNs and PTs will be changed from “Congenital abnormal fusion of X bone” to “Congenital abnormal fusion involving X bone.”

These single-bone concepts serve as groupers for congenital abnormal fusion concepts in which the specified bone is represented in the concept model.

Note: this is the first step in a series of changes to Congenital abnormal fusion and synostosis concepts to address modeling and classification errors.

These concepts can be used to represent the type of stain utilized on a slide in digital pathology. Examples include "Estrogen receptor antibody brown chromogen immunohistochemistry (qualifier value)|" and "Programmed cell death 1 ligand 1 monoclonal antibody 22c3 brown chromogen immunohistochemistry staining technique (qualifier value)." Editorial guidelines will also be developed for this area.

AJCC Cancer Staging Updates

Following a collaboration meeting with the American Joint Committee on Cancer (AJCC), the TNM stage groupers and grade groupers will be replaced with generic stage and grade groupers, aligning with the expected data flow. The TNM-specific groupers use shared code values that are not identified separately by T, N or M, and so are already functioning as generic groupers. As agreed with AJCC, they will be replaced with true generic groupers, since T, N and M values are identified separately in the data.
