IHTSDO NurSIG: Alliance for Nurisng Informatics

The IHTSDO Nurisng SIG is a member of the Alliance for Nursing Informatics (ANI) which is now an international organization. Currently, I (Judy Warren) serve as our liaison. IHTSDO NurSIG: ANI: HIMSS Board of Directors has approved the HIMSS Position Statement on Transforming Nursing Practice through Technology Informatics.

Greetings ANI Community members, theI am pleased to announce that the HIMSS Board of Directors has approved the HIMSS Position Statement on Transforming Nursing Practice through Technology Informatics.( HIMSS is also an international organziation and a member of the ANI.) As you all know, nurses are key leaders in developing the infrastructure for effective and efficient health information technology that transforms the delivery of care. The landmark report from the Institute of Medicine& Robert Wood Johnson Foundation, Future of Nursing: Leading Change, Advancing Health Report, determines criteria to transform the nursing profession, leading to new roles and leadership positions for nurses in the redesign of the health care system (this report may be found at http://thefutureofnursing.org/IOM-Report, Though USA-centric, the report may be of interest to all countries as many of the issues are the same). The HIMSS Board of Directors, effective June 17, 2011, has approved the position statement that describes how we must transform nursing practice through technology and informatics. The position statement was developed by leaders from the HIMSS Nursing Informatics Committee, which now represents over 2,900 members, and is intended to not only serve the nursing profession but also the broader healthcare industry and HIMSS membership at large. In the UK we are developing guidance and content for implementers af care planning functionality. We are proposing some additions to the current goal achievement findings. The existing international content is shown below: 390800000 Goal achievement finding 390802008 Goal achieved 390801001 Goal not achieved 425372005 Incongruent personal goals The following are suggested as additions to this list, and we would like to invite feedback before submission. Direct email to me today has suggested Do you also need "goal not agreed" ? I would make sure that the 'progress towards' concepts are constructed consistently, either: Making progress towards goal Not making progress towards goal Or Progressing towards goal Not progressing towards goal Look OK to me otherwise Consequently the currently list of addition requests is likely to be * Goal not agreed * Goal agreed * Progressing towards goal * Not progressing towards goal * Goal partially achieved * Divergence from goal - (wording of this is not universally liked in informal discussions), contributions as to more clear ways to articulate this concept would be welcomed. As would expressions of support??

Thank you Elaine As we have discussed since kindly posting this, "variance" may raise as many issues as it potentially solves, but is likely to be a legitimate addition particularly for those using SNOMED CT in care pathways rather than more traditional care planning. One thing is that variance is a super-type of both not progressing towards goal and the “divergence from goal”. Potentially it could even include the progressing towards goal in some circumstances as variance is often measured against time, whereas many traditional goals are often simply an identified end-point. I have posted to the group to invite any more suggestions and/or comments which would be welcome. Many thanks.

Am I correct in understanding this would be a suggested change to the concept model as a linkage concept, rather than relying on the information model? The wording did not seem to match up with a goal status in itself. This is certainly an area that would benefit from consideration as we move to a more outcome measure focused agenda in terms of healthcare delivery. Anne will no doubt update me on her return or feel free to contact me directly off-line to clarify.

Whilst nursing represent the largest part of the healthcare workforce it often has an under-represented voice at a senior level. The IHTSDO has a specific voice for Nursing in the Nursing Special Interest Group, however there are other much smaller professional groups in healthcare that are not represented formally within the IHTSDO. This paper suggests widening the scope to “Multidisciplinary care SIG” or similar, if retaining Nursing as the focus is considered important perhaps “Nursing and multidisciplinary care SIG”.

Following SIG discussion in Feb, I provided the feedback below and the attached annotated document to the consultant terminologist working on this topic. Thanks for all contributions. 1. We would suggest that there is a clear statement of the information requirement that this content is supporting so that we can all assess whether the ‘limitation of scope’ is a real problem or the content just looks limited. That would also help to support advice on how to meet the information need using a good information model rather than further precoordination. 2. The three hierarchies listed in section 3.4 are all problematic and it is not clear that any of them are needed. So we can’t support the proposal that a new model should support modelling the three patterns. Since the content mostly came from the UK it would be appropriate to ask Jeremy Rogers to run a query on how frequently these codes are used in UK systems – to begin to understand the impact of any proposed changes. 3. If any of the content is required, the concept names need to be reviewed for international use: district nurse etc may be specific to one country. IHTSDO member countries need to be surveyed to see if this is a multinational phenomena or content moved to extension.
