International Extensions and Package manager

Really just making this a note to be a topic at the October meeting as I close on of our tickets [MSSP-3494] Inclusion of International Extensions into National Release Edition. - SNOMED International JIRA

But I'd be really interested to hear how any other MS users are currently making International Extensions available for users?
For example any of these listed in MLDS. 

But specifically we've got users interested in the ICNP package, and also the LOINC extension

For the packages that are just refsets, you could probably get away with just bolting them onto the RF2 (no classification consequence).
But with FHIR and the move to terminology services, there are technical challenges like what is the URI or Edition being queried? The MLDS for these and Edition (composition) becomes complex also.
